It is time to submit (or review and correct) your Health Insurance Information for the Spring 2014 semester. If after review, the insurance policy is found to be valid under the "3 out 4" rule, your Health Administration Fee for the Spring 2014 semester will be waived. (Please Note this process can take up to 10 business days). This document is required of all students who are attending Bryan for SP-14 semester. Please take a few minutes to review and submit this on-line form. You will need your insurance card in order to complete the form.
This on-line form will ask you for a token. Starting December 2 2013, all New and Returning students should have received and email containing their token and link for the Online Insurance form. Also the token can be found on eStudent under ‘Grades’ and it is called Student ID. The token begins with an A0.... and has more digits that completes the number. You can use this ID/token to log-in and update your insurance information at any time.
You will be able to check the status of this process on your eStudent accounts under a document called “Student Insurance”. If your insurance document on eStudent has been marked ‘rejected’ there are 2 explanations: 1) your insurance does not qualify for the waiver. or 2) the information submitted is either incomplete or incorrect and the PolyClinic will contact you about this. Most of the time it is something simple, like a question left unanswered or policy numbers transposed. Any insurance documents that continue to be marked ‘Incomplete’ or ‘Rejected’ will result in a $200 Health Administration charge on your billing account beginning with Jan 1, 2014.
